人气极旺的名棒球手一郎（一郎 饰）从美国回到日本，大宋引起极大轰动。古畑任三郎（田村正和 饰）和今泉慎太郎（西村雅彦 饰）都是一郎的粉丝，大宋得知向岛音吉（小林隆 饰）在一郎的酒店担任护卫，二人缠着向岛帮他们索要签名，更令他们惊奇的是向岛竟然是一郎的哥哥。可是脸上永远挂着笑脸的向岛也有他的烦恼，长久以来他一直被有着黑社会背景的男子郡山繁（今井朋彦 饰）勒索，此番更要想法筹备1000万日元。走头无路的向岛打算铤而走险杀掉对方，而与他感情笃深的一郎则要与哥哥共同进行一场完美谋杀案。命案再起，精心策划和布置的现场，古畑刑警最大的挑战……本片为新春完结篇第二部。
